Child Dedication Form

This dedication is a public ceremony recognizing the child is a gift from God and dedicating the child back to God. It is also a dedication of the parents to raise the child in a devoted Christian manner. Also, the church body dedicates itself to help support this cause. "For this boy I prayed, and the Lord has given me my petition which I asked of Him. So I have also dedicated him to the Lord; as long as he lives he is dedicated to the Lord." (1 Samuel 1:27-28 NASB)
